What your first back and spine session involves

The first appointment is mostly assessment. Your physiotherapist will ask how the pain started, what makes it worse through the day, whether it travels into the buttock or leg, and how it affects sleep and work. That history usually narrows the cause more than any single test. You will then be asked to bend, arch and rotate so we can see which directions are restricted or painful, and we check strength, reflexes and sensation in the legs when nerve involvement is suspected. Bring any X-ray, MRI or CT report you already have, though treatment is guided by how your back actually behaves rather than by the scan alone.

How a course of treatment usually runs

Most people with mechanical back pain attend two or three times a week at first, then taper as symptoms settle. An acute episode often eases meaningfully within two to four weeks, while long-standing pain or a disc problem with leg symptoms generally needs a longer programme measured in months. Sessions combine hands-on mobilisation to restore movement with exercise that rebuilds the deep trunk muscles supporting the spine. Recovery is rarely a straight line, and a flare after a busy day does not mean the programme has failed.

What helps between appointments

  • Keep moving within comfort. Prolonged bed rest slows recovery for most back pain.
  • Break up long periods of sitting. Standing and walking for a few minutes each half hour matters more than any single perfect posture.
  • Do the prescribed exercises little and often rather than one long session.
  • Adjust your workstation so the screen is at eye level and your feet reach the floor.
  • Lift with the hips and knees, keep the load close, and avoid twisting while carrying.

When to see a doctor first

Physiotherapy is not a substitute for medical diagnosis. Seek urgent medical attention rather than booking physiotherapy if you have numbness around the groin or inner thighs, loss of bladder or bowel control, progressive weakness in a leg, unexplained weight loss with back pain, fever alongside spinal pain, or back pain following a significant fall or accident. These are uncommon, but they need a doctor before any hands-on treatment begins.
