What your first neck and shoulder session involves

Neck and shoulder symptoms overlap constantly, so the assessment covers both regions even when the pain feels local. We look at how far the neck turns and side bends, how the shoulder blade moves as the arm lifts, and whether pain refers into the arm or up into the head. Grip strength and sensation are tested when nerve irritation is suspected. Desk workers are also asked about screen height, chair setup and how many hours pass between breaks, because the pattern of use is often more informative than the site of the pain.

How a course of treatment usually runs

Simple mechanical neck pain often responds within a few weeks of twice-weekly treatment. Frozen shoulder is different and needs honest expectations: it moves through a painful phase, a stiff phase and a thawing phase, and the whole process commonly takes many months regardless of treatment. Physiotherapy through that period is aimed at protecting range of movement, managing pain and keeping the surrounding muscles working, so that recovery is as complete as possible once the stiff phase passes. Rotator cuff problems usually respond to a progressive loading programme rather than rest.

What helps between appointments

  • Set the top of your monitor at eye level and pull the chair in so you are not reaching forward.
  • Take a short movement break every half hour rather than one long break at lunch.
  • Support the arm on a cushion when sitting if the shoulder aches at rest.
  • Use one supportive pillow that fills the gap between ear and shoulder instead of stacking several.
  • Carry bags across the body or in a rucksack rather than on one shoulder.

When to see a doctor first

Arrange medical review rather than physiotherapy if neck pain follows a road accident or fall, if you have severe unremitting night pain, if there is progressive weakness or wasting in the arm or hand, if you feel dizzy or unsteady with neck movement, or if neck pain comes with fever, unexplained weight loss or a history of cancer. Sudden chest, jaw or left arm pain requires emergency care, not a physiotherapy appointment.
